Register for Activity

HOW TO REGISTER FOR CE & REC PROGRAM

  1. Find an activity!
  2. Register online or at Menomonie Field House, 1715 5th Street W, Menomonie, WI 54751
  3. All activity fees must accompany the registration form. Payments can be made in cash, check or by card. Make checks payable to Menomonie Area School District.
    • Participants registering for an activity that are under the age of 18 must have a parent signature on the registration form.
    • Please note that your registration confirms your commitment to the activity. Refunds will only be given if cancellation is received 1 week prior the start of activity or in the event the activity is cancelled.
    • Have a GREAT TIME! While this is not essential, I am hopeful that you will be fully satisfied with the enrichment of your activity!

PRE-REGISTRATION

Pre-registration is required for all Community Education & Rec activities.

  • ON-LINE REGISTRATION: Go to Rec Desk
  • MAIL: Mail the form with your payment to: Menomonie Area School District, Attn: MHS Field House, 1715 5th Street W., Menomonie, WI 54751.
  • WALK-IN: Office Hours are Monday thru Friday 8:30 a.m. to 3:00 p.m. The MHS Field House Department office is located in the FieldHouse area of the Menomonie High School. Please use Main High School Entrance.
  • DROP-OFF: Drop off form with payment at Menomonie High School office, Attn: Field House

*Confirmations will not be sent but feel free to call us to confirm your registration.

LATE FEE REGISTRATION

All participants that are not signed up prior to the start of the activity, will be assessed a late fee of $10 per registration.

Don't forget to register early!!
